Swasthya Sathi Scheme Eligibility Criteria
i) Resident of the state – Only legal and permanent inhabitants can register for this project.
ii) BPL applicant – It is pertinent that the applicant falls in the BPL category. If he/she is registered under RSBY, then the individual can apply.
iii) Medical and SHG workers – All such members, which are associated with ASHA, ANM, and ICDS will get the perks of this scheme. The same will be applicable to self-help group members.
iv) Civil volunteers – If any person works as a civil volunteer, then he/she will attain this medical insurance coverage.
Swasthya Sathi Scheme in West Bengal Documents necessary
i) Residential papers – It is important that the applicant produces his/her legal domicile documents.
ii) BPL certificate – All applicants must furnish their BPL certificates.
iii) Registration certificate from medical associations and SHGs – The SHG and medical organizations’ registration certificate is necessary.
iv) Civil volunteer certificate – The candidates need to submit an authorized document that has been issued by the WB Government.
Main Features of the Swasthya Sathi Scheme:
> Basic health cover for secondary and tertiary care up to Rs. 5 lakh per annum per family.
> Up to Rs 1.5 Lakh through Insurance Mode and beyond 1.5 lakh to 5 Lakh through Assurance Mode.
> The scheme was rolled out from 01.02.2017. National Insurance Company was an insurance partner in 9 Districts and United India Insurance Company in 11 Districts up to 28.02.2018. The scheme was implemented under assurance mode for the month of March 2018.
From 01.04.2018 to 15.01.2020, Bajaj Allianz selected as Insurance Partner in 18 Districts, and from 01.04.2018 to 31.12.2019, IFFCO TOKYO selected as Insurance Partner in 5 Districts. The service of 5 districts has been extended by Swasthya Sathi Assurance Mode up to 15.01.2020.
> The service is continued by the new Insurance Company from 16.01.2020. National Insurance Company is selected as Insurance Partner in 6 Districts (Cluster-I), Oriental Insurance Company is selected as Insurance Partner in 9 Districts (Cluster-II), United India Insurance Company is selected as Insurance Partner in 8 Districts (Cluster-III).

> There is no cap on the family size and Parents from both the Spouse are included. All dependent physically challenged persons in the family are also covered.

Steps to Download Swasthya Sathi Scheme Application Form
1) Visit the nearest government hospital to apply for the scheme. the enrollment will take place in offline mode
2) Applicants can register through agents at the village medical centers nearby.
3) Agents assist the people in the offline application process.
4) After that, on the application process and applicant must attach the photocopies and originals of all documents mentioned in the scheme guidelines as the enrollment agent will verify the required documents.
5) The enrollment agent can help the people to log in and fill in the registration form on behalf of the candidate.
6) Once the enrollment form is filled and submitted, the site will generate an ID and password for the candidate.
7) Now, the applicant can officially log in on the scheme site with the ID and password.
8) The Swasthya Sathi Smart Card will be generated via the official portal. After logging in, the applicant can acquire this digitized card.
